Athlete Waiver Form

I, the undersigned parent/guardian (or athlete if 18+), acknowledge and agree to the following:

1. Assumption of Risk. I understand that wrestling is a contact sport that involves inherent risks of physical injury, including but not limited to sprains, fractures, concussions, and other injuries. I voluntarily assume all such risks on behalf of the above-named athlete.
2. Release of Liability. I hereby release, waive, and discharge Peninsula Wrestling Club, its coaches, staff, volunteers, and affiliated parties from any and all claims, demands, or causes of action arising out of or related to the athlete's participation in Peninsula Wrestling Club activities, including injuries caused by negligence.
3. Medical Authorization. In the event of an emergency in which I cannot be reached, I authorize Peninsula Wrestling Club staff to seek and consent to emergency medical treatment for the above-named athlete. I agree to bear all costs of such treatment.
4. Photo/Media Release. I authorize Peninsula Wrestling Club to use photographs, videos, or other media featuring the athlete for promotional, educational, or social media purposes, without compensation.
5. Code of Conduct. I agree that the athlete will conduct themselves with respect toward coaches, teammates, and opponents at all times, and understand that misconduct may result in removal from the program.
6. Accuracy. I certify that all information provided in this form is accurate and complete to the best of my knowledge.
